2-1-1 San Diego
On June 13th, 2016, 2-1-1 San Diego showed us its state of the art resource and information hub that connects people with community, health and disaster services through a free, 24/7 stigma-free confidential phone service and searchable online database. 2-1-1 serves the entire population of the County. Patient Navigator Programs
On November 9th, 2015, Sacramento Covered graciously presented us with an explanation of their highly successful Patient Navigator Program. Through this program Sacramento Covered helps consumers understand and properly utilize their insurance, staying healthy and out of the emergency room. Their work is funded by insurance plans wishing to minimize unnecessary usage of the Emergency Room, which can be costly and divert resources from people with critical emergencies.
Patient Navigator Programs can be a great way for outreach and enrollment organizations to partner with hospitals and health plans to improve patient outcomes while generating sustainable revenues for the organization. A special presentation by Community Health Initiative of Napa County, Solano Coalition for Better Health and the California School Boards Association on how enrollment organizations can connect with their local school districts to make sure all children are enrolled. Through the right agreements and privacy procedures, enrollment organizations can use school enrollment forms to gather information on families who may need help with health coverage.

Tools for Consumers on Quality of Health Plans and Resources on Access Issues: This webinar demonstrates how to navigate the online California Office of the Patient Advocate Health Plan Report Cards. The OPA staff show how to find the star ratings and scores on the care provided for diabetes, asthma, and other conditions such as vaccinations and cancer screenings. The patient experience data is also shown on measures such as doctor-patient communication and ease for getting appointments. This webinar also covers how we can help consumers when there are problems enrolling them in the right coverage or when there are issues accessing care.
