Patient Navigator Program




On November 9th, 2015, Sacramento Covered graciously presented us with an explanation of their highly successful Patient Navigator Program. Through this program Sacramento Covered helps consumers understand and properly utilize their insurance, staying healthy and out of the emergency room. Their work is funded by insurance plans wishing to minimize unnecessary usage of the Emergency Room, which can be costly and divert resources from people with critical emergencies.

Patient Navigator Programs can be a great way for outreach and enrollment organizations to partner with hospitals and health plans to improve patient outcomes while generating sustainable revenues for the organization. Find out more and watch the live recording here.
